В ходе исследования проанализированы публикации, размещенные в российских и зарубежных базах данных (eLibrary, PubMed, Google Scholar, UpToDate) преимущественно за последние 5 лет по следующим ключевым словам: «артериальная гипертензия», «детское ожирение», «поражение органов-мишеней», «hypertension», «childhood obesity», «target organ injury». При анализе данных учитывали все публикации, посвященные сочетанию детского ожирения и артериальной гипертензии, их влиянию на поражение органов-мишеней, методах диагностики и лечении.</p></sec><sec><title>Результаты</title><p>Результаты. Распространенность артериальной гипертензии и ожирения в детской популяции растет, несмотря на усилия общественного здравоохранения. Артериальная гипертензия и ожирение способствуют выработке провоспалительных цитокинов, которые активируют ренинангиотензин-альдостероновую систему и симпатическую нервную систему, что приводит к негативным последствиям в регуляции АД и функции почек. При повышенном АД могут наблюдаться неблагоприятные изменения в работе сердца, сосудов, почек, сетчатки глаза и нарушаться нейрокогнитивные функции. Результаты последних исследований свидетельствуют о достижениях в области терапии артериальной гипертензии и ожирения у детей, что способно снизить риск повреждения органов-мишеней в педиатрической популяции.</p></sec><sec><title>Выводы</title><p>Выводы. Артериальная гипертензия и ожирение оказывают значительное влияние на ПОМ у детей. Мероприятия по нормализации АД и терапии детского ожирения способны уменьшить гипертрофию левого желудочка, улучшить показатели систолической и диастолической функции, а также функцию почек. Надлежащий скрининг и лечение этих заболеваний могут снизить потенциальное негативное влияние на сердечно-сосудистую систему в будущем.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study data on the relationship between childhood obesity and hypertension, their effect on target organ damage (TOD), as well as to analyze available therapeutic options.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We carried out a review of scientific publications presented in Russian and foreign databases (eLibrary, PubMed, Google Scholar, UpToDate). The review depth was five years; the search keywords were «arterial hypertension», «childhood obesity», «target organ damage» both in the Russian and English languages. When analyzing the retrieved data, all publications on the combination of childhood obesity and hypertension, their effect on target organ damage, diagnostic methods, and treatment options were considered.</p></sec><sec><title>Results</title><p>Results. Despite public health efforts, the prevalence of hypertension and obesity in the pediatric population is constantly increasing. Hypertension and obesity contribute to the production of pro-inflammatory cytokines that activate the reninangiotensin-aldosterone system and the sympathetic nervous system, which leads to negative consequences in the regulation of blood pressure and kidney function. Increased blood pressure triggers adverse changes in the functioning of the heart, blood vessels, kidneys, and retina. Neurocognitive functions may also be impaired. The results of recent studies indicate advances in the treatment of hypertension and obesity in children, which can reduce the risk of damage to target organs in the pediatric population.</p></sec><sec><title>Conclusions</title><p>Conclusions. Arterial hypertension and obesity have a significant impact on TOD in children. Measures to normalize blood pressure and treat childhood obesity can reduce left-ventricular hypertrophy, improve systolic and diastolic function, as well as kidney function.
